Moving Home Minnesota assists individuals receiving Medical Assistance (MA) to transition from institutional settings back into community living. This program covers costs not included under MA or MA-Waiver benefits, offers counseling, and ensures a smooth shift to community life.
Commonly referred to as “Money Follows the Person,” Moving Home Minnesota is the state’s adaptation of this federal initiative.

Services from Moving Home Minnesota extend for up to one year post-transition into the community. For those also receiving MA and MA-Waiver program benefits, Moving Home Minnesota will cover additional services not paid for by these programs.
The primary objective of Moving Home Minnesota is to aid individuals in moving out of institutional environments. Eligibility requires being an MA recipient and having resided in an institution for a minimum of 60 days at the time of application.
Facilities under different names may also meet the criteria for institutions.
