Relocation service coordination is a type of case management to help people who want to move out of certain institutions into the community. RSC helps people plan and arrange for the services and support they need to live in the community.
If you choose to move out of an institution, you or someone acting on your behalf must make a request to the locally county agency or tribe. The county will assign a case manager to visit you within 20 working days of your request and give you a list of names of relocation service coordination providers in your area.
You may also request RSC as part of a long-term care consultation (LTCC) if you are under age 65 and live in a nursing facility.
You are eligible for relocation service coordination if you meet all of these criteria:
